Expedition Team Members’ Area

Hello Team Members! We are very excited to have you join us as a Registered Team Member. The red links on the right side of this page will allow you to submit your required personal, emergency contact, and travel information via online forms. We utilize a secure transmission method that only authorized 2041 staff will be able to access. Important: once you press ‘update’ at the end of each form, the information will be submitted directly to our database. You can “redo” your forms at any time – for example if your flight information changes.
Beginning November 1, you will be assigned a user name and password. You only need to login once to fill out all the forms. When you are done with your forms, please make sure to use the logout link at the top of the page you are visiting. All forms must be completed and submitted at least 30 days prior to the start of your expedition.
The Web Bio Questionnaire asks for a short biography and picture that will be posted to the Team Members’ area of our website and will be accessible by all expedition participants and the public. We ask for you to submit this questionnaire sooner rather than later, as this information can be very helpful in getting to know your fellow “expeditioners” before you start your adventure. Please make your image no larger than 100k and include only from the waist up, no sunglasses on and only you in the photo.
You will also be required to print, fill in and sign the Terms and Conditions and Medical Forms located on the right and scan and email them back to the 2041 office. Please make sure to make an appointment with your physician as soon as possible to allow for these forms to be completed and submitted to 2041 at least 30 days prior to the start of the expedition.
